Office policies
New patients are triaged by urgency after a careful review of your appended medical information. Our office is unable to respond to give detailed information about the waiting list.
Our office will contact you to book your initial appointment. Your appointment has to be confirmed via telephone in speaking with the medical office assistant within 24 hours of a message being left.
Due to the high volume of patients on the waitlist, our office has a cancellation policy in place and we require 24 hours advance notification by phone.
Extenuating circumstances, sickness, and weather conditions are excused
Patients failing to attend an appointment without cause will be returned to the care of their referring practitioner
Any medical forms to be completed by the physician will be done for a fee of $30. Forms to be given to the medical office assistant at the time of appointment.
New patients should present 15 minutes prior to the appointment time to fill out a history form. Please bring a complete medication list including dosages.
Initial appointments are booked for 30 minutes. Be prepared for a complete history and physical exam.
Follow up appointments are booked for 15 minutes. One issue per visit will be addressed. If further issues exist, a follow up appointment can be booked at a later date.
Prescription renewals can be completed by contacting the office up to six months after an initial appointment or follow up visit.
If it has been six months from your initial appointment, or follow up visit, a new referral from your family physician has to be completed.
Due to the high volume of patients, Dr. Baikie cannot return phone calls. A follow up visit can be arranged to address any concerns.
